Fifty eight cases of congenital preauricular fistulas surgically repaired at the department of ENT/HNS of Taegu Fatima Hospital during the last 5 years.(March, 1987 to Feb, 1992) There cases were retrospectively studied and showed the following results. 1) Of 58 patients, 33(53%) were women and 25(43%) were men with a slight female predominent sex ratio of 1.3 : 1. 2) surgical age distribution in order of frequency, the peak age incidence was in the 10~20 years age group(31%) and 1-5(29%), 5-10(17%), over 20(15%) and below 1(6%). 3) In 14(25%) patients, fistulas were found bilaterally and in 44(75%), unilaterally. Of 44 unilateral fistulas, 25 were on the left side and 19 on the right. 4) The location of the fistulous openings were found in 42(65%) at the anterior portion of the crus of helix and upper portion of the tragus, 15(23%) at the margin of helix, on the crus of helix in 4(6%), within external auditory canal in 1, on the antitragus in 1 and anterior to the lobule in 1. 5) The most frequent location of cyst were at the anterior portion of the crus helicis and upper portion of the tragus(36 ears/54%). The second most common site was the margin of the helix(24 ears/36%). In 3 ears the cyst was found within the cavum concha and in 2 on the posterior surface of the auricle and even anterior to lobules in 1 case.
