Intralaryngeal cysts with laryngeal microsurgery.

ABSTRACT

Laryngeal microsurgeries in the treatment and diagnosis of the laryngeal diseases have been more increasingly performed, and laryngeal microsurgical techniques have adbanced and gained widespread acceptance. Laryngeal cysts can be easily recognized and simply managed using microsurgery. The saccular cysts appear when obstruction of the orifice of the saccule occurs, with resultant dilatation by glandular secretions. The ductal cysts, known as mucous retension cysts, result from retension of mucous in dilated collecting ducts of the submucosal glands and can be found anywhere in the larynx. The epidermoid cysts are not rare, and are found in the free margin of the vocal cord. In the present series, 24 cases of intralaryngeal cysts treated from 1988 to 1992 are reported. In the clinical evaluation, the epidermoid cysts are considered the most frequent type of intracordal cysts, and the intralaryngeal cysts are not easily diagnosed, especially the ductal cysts. Pathologically saccular cysts and ductal cysts are lined with mucosal epithelium, but epidermoid cysts are lined with squamous epithelium. The intracordal cysts showed diminished vibratory patterns on stroboscopic examinations, so stroboscopy is the recommended diagnostic tool. An ideal treatment for intracordal cysts is to enucleate the cyst with minimal surgical intervention.
