A Case of Complete Branchial Fistula of the Neck

ABSTRACT

Persistence of branchial remnants is fairly common, however, complete branchial fistulas in the neck seem to be rare. A 13-year-old boy visited our Out Patient Department on September 2, 1969 because of continuous discharge from a small opening in the lower neck. External examination revealed a very small and inconspicuous opening situating on the anterior border of the sternocleidomastoid muscle at the level of lower third of the neck, and small amount of mucous discharge from the opening could be seen especially on swallowing action. X-ray study with rediopaque substance injection through the opening demonstrated a distinct fistulous tract extending all the way up to the region of the supratonsillar fossa. In surgical excision, the so-called stepladder method (developed by Hamilton Bailey) was applied and very satisfactory result could be obtained. The diagnosis was confirmed by histopathological study.
