Penetrating Wound of the Soft Palate Accompaning with Rupture of the External Carotid Artery

ABSTRACT

We experienced 6 years old boy who had suffered from perforation of the left lateral part of the soft palate and the lateral wall of the pharynx accompaning with rupture of the external carotid artery. On physical examination, severe swelling of the left lateral aspect of the neck, perforation wound of the left side of the soft palate with gauze packing and severe swelling of the left lateral wall of the pharynx were noted. In the perforation wound, about 3cm of wooden bar (hot dog bar) was found and removed out. Under the impression the rupture of the external carotid artery because of massive arterial bleeding, carotid angiography was performed after gauze packing in the perforation wound. Angiogram revealed extravasation of the contrast media around the external carotid artery. Exploration of the left carotid arterial system was done and there was repture of the proximal portion of the lingual artery arising from the external carotid artery. And so lateral repair of the ruptured artery was done.
