A Clinical Study of Hearing Loss after Head Trauma

ABSTRACT

We studied 52 patients who had been hospitalized in the presbyterian Medical Center, after had injury from May, 1977 to February, 1982. 23 temporal bone fractures, 22 skull fractures without temporal bone fracture and 7 head trauma without skull fractures had no history of ear trouble and noise exposure. All The patients underwent a through ENT examination, X-ray films of the skull. Pure-tone audiograms (ISO) were also taken, as well as speech discrimination tests. The results were as follows : 1) Of the 52 patients, 36 patients (69%) were men and 16 patients (31%) were women. 2) Most of them were injured in an automobile accident. 3) A hearing test was done on 1st to the 45th day after head trauma, but 34 patients (65%) were tested within 10 days. 4) On otologic symptoms and findings 1) a) Mard of hearing was 35 patients (67%), b) Ear bleeding was 23 patients (44%). 2) Facial nerve paralysis was observed in 4 cases with temporal bone fracture, and in 1 case with skull fracture without temporal bone fracture. 5) On temporal bone fracture (1) Longitudinal fracture was 17 cases (74%), (2) Transverse fracture was 5 cases (22%), (3) Mixed fracture was 1 case (4%). 6) On hearing loss of temporal bone fracture (1) Longitudinal fracture ; 6 cases were conductive hearing loss (35%), (2) Transverse fracture ; 4 cases were sensorineural loss (80%), (3) Mixed fracture ; 1 case was sensorineural hearing loss (100%). 7) Unilateral hearing loss was 25 cases (48%), Bilateral hearing loss was 6 cases (12%). 8) (1) 15 of the 17 cases of conductive hearing loss were mild (10 cases) and moderate (5 cases) hearing loss. (2) 8 of II case of sensorineural hearing loss were mild (4 case) and profound (4 cases) haring loss. (3) 8 of the 9 cases of mixed hearing loss were mild (2 cases), moderate (3 cases) and moderately severe (3 cases) hearing loss. 9) Follow up, (1) follow up of 18 cases of hearing loss A) 6 cases of spontaneous recovery during 1 months of follow up a) 4 cases of spontaneous recovery during 1 months of follow up, b) 1 case of sensorineural hearing loss, c) 1 case of mixed hearing loss. B) After observation during 2 months, ossiculoplasty was done on 1 case. Then hearing improvement was shown. C) Ossiculoplasty was not performed on 1 case. (2) 2 of the 5 cases of facial palsy improved spontaneously during 1 month.
