Management of Bronchial Foreign Body by Ventilating Bronchoscopy

ABSTRACT

The authors present detailed technique of ventilating bronchoscopy based on recent experiences with clinical analysis of 87 patients who were treated by ventilating bronchoscopy out of 93 airway foreign body patients during the period of 1980 to 1983. Statistical analysis was also performed upon the 93 airwary foreign body cases. 1) Among the 93 cases, 82.8% were under the age of 3. M : F was 2.3 : 1. 2) Success rate of foreign body removal by ventilating bronchoscopy was 97.7%, 2 cases were treated by thoracotomy. 3) Nuts are by far the most common airway foreign bodies comprising 64.5% of total cases. Plastics were 16%, metals were 11% and foods were 6%. 4) Abnormalities in chest X-ray were found in 85 cses among 93(91.4%). Foreign body shadows were found only in 12 cases. 5) Duration of procedure was within 20 minutes in 56.5%. 6) Complications after ventilating bronchoscopy were found in 16 cases(18.4%). They were pneumonia (7), atelectasis (5), atelectasis with pneumothorax (1), pneumothorax (1) and cardiac arrest (2). One of the cardiac arrest cases was recovered by CPR but the other was expired due to preexisting severe hypoxia. 7) 44 foreign bodies were found in right lung and 30 were in left. 8 were found in trachea.
