Improving the Surgical Access in Intranasal Ethmoidectomy by Mobilizing the Nasal Septum

ABSTRACT

A method of septal mobilization, by way of the maxilla-premaxilla approach modified by the complete transfixion incision, was applied in 7 cases of intranasal ethmoidectomy for improved visualization and access to the ethmoidal labyrinth. Certain parameters of the surgical field were evaluated and CT was obtained along the planes of the surgeon's view to the anterior and posterior ethmoidal cells in order to estimate how well the operative field could be accessed by this procedure. The described procedure affords better visualization and access to the ethmoid in intranasal ethmoidectomy through the enlarged naris as a surgical inlet ; by the improved visual angle of the surgeon to the ethmoid ; with the increase in upward mobilization of the naris ; and through the transfixion incision as another surgical route.
