A Clinical Analysis of Allergic Rhinosinusitis

ABSTRACT

Allergic rhinitis may develop sinusitis more frequently because of contribution to obstruction of the sinus ostia due to mucosal edema and increased secretion, and decreased self-cleansing mechanism due to ciliary dysfunction. A clinical analysis was performed on selected allergic rhinosinusitis group among 3101 patients, who complained of nasal problems except patients with nasal polyp, cancer in nasal cavity or paranasal sinuses, structural nasal obstruction(ex. ; nasoseptal deviation), or mechanical nasal obstruction(ex. ; foreign bodies in nasal cavities etc.), during the period from June 1987 to June 1989 in Otolaryngologic Department of National Medical Center. The obtained results were as follows ; 1) On 3101 patients who complained of nasal problems during that period, allergic rhinosinusitis group was 223(7.2%) patients. 2) By age distribution, 2nd decade<946 patients(32.9%)> were most frequently affected in the non-allergic rhinosinusitis group, 3rd decade<64 patients(28.7%)> were most frequently affected in allergic rhinosinusitis group. Average ratio for male to female was same in both groups as 3 : 2. 3) By monthly distribution, Jan., July and Aug. were highly prevalent months in non-allergic group, but that showed even distribution in allergic group. 4) The patients who had the duration of illness of below 6 months were 600 patients(20.8%) among the non-allergic group, and were 9 patients(4.0%) among the allergic group. And so the longer duration illness which was above 6 months was more prevalent in allergic group. 5) The patients who had abnormal PNS(paranasal sinus) X-ray findings were 1414 patients(49.1%) in non-allergic group, and were 127 patients(57.0%) in allergic group. And in the PNS X-ray findings, bilateral involvement of maxillary sinus was revealed as 95 cases(74.8%) in allergic group, and as 679 cases(48.0%) in non-allergic group. 6) In the allergic rhinosinusitis group, abnormal PNS X-ray findings were much more revealed in positive skin test to house dust, house dust mite or fungi(perennial allergen) than to pollens(seasonal allergen). 7) In the allergic rhinosinusitis group, serum IgE PRIST(Paper Radio Immuno Sorbent Test) level and blood eosinophil count were not related to the abnormal changes of PNS X-ray or severity of the sinusitis on PNS X-ray findings.
