Design of Pectoralis Major Myocutaneous Flap for Reconstruction of the Tonsillar Area

ABSTRACT

The oropharyngeal defect after radical removal reveals valley shaped one with loss of a wide mucosal area. But its space is quite small. Using pectoralis major myocutaneous flap to this defect is sometimes difficult due to its natural figure of bulkiness. The author modified design of the flap from oval to kidney shaped bilobular one. The advantages of this modification are easier folding of the flap, smoother configuration. This article reviews patients modified flap design in 14 undergoing pectoralis major myocutaneous flap in carcinoma of the oropharynx. Conventional oval shaped flap was used in 8 patinets and bilobular flap was used in 6 patients. Total loss of the flap was occurred in one case of the bilobular falp, but other complications were more common in oval flap. Bilobular modification of the pectoralis flap was more easier to manipulate so it fits more safely to the valley shaped oropharyngeal defect.
