The Management of Epistaxis by Suction Eletrocautery

ABSTRACT

Epistaxis is common and often emergency in otolaryngology. Treatment is usually by anterior nasal packing. There is often significant problem associated with nasal packing. Not only does it cause considerable discomfort such as nasal obstruction, rhinorrhea and sneezing but also cause toxic shock syndrome. This require prophylatic medication and revisit to remove packed material. We study the effectiveness of suction electrocautery. From January 1993 to June 1994. 68 patient were selected. Twenty five patient were treated by suction electrocautery and endoscope, other treated by antrior nasal packing. The result were as followed 1) There was no significant difference in effectiveness of each tratment(p<.05). 2) There was no significant difference in interval of recurrence between two group(p<.05). 3) It was lesser effective in treatment of posterior nasal packing(35.7%) than other site(78.4%) by nasal packing. 4) We successfully control 2 diffcult case with posterior nasal bleeding by suction electrocautery. We belive that suction electrocautery is equally effective as compared with antrerior nasal packing, but the former is more simple and comfortable than the latter. We recommend the suction electrocautery and endoscope as treatment of choice for epistaxis especially for troublesome posterior nasal bleeding.
