<trans-abstract xml:lang="en"><p>Congenital stapedial fixation is a type of conductive hearing loss in which impairment of the sound-conduction mechanism is caused by congenital or acquired fixation of the stapes. It is genetically heterogeneous, but it has been recently reported to be also caused by mutations in the <italic>Noggin (NOG)</italic> gene. The authors have experienced a 6-year old boy with bilateral mixed hearing loss, who has a family history of hearing loss. No stapedial reflex was observed, and temporal bone CT showed no abnormality in the middle ear, as well as in the ossicles. Genetic study revealed novel <italic>NOG</italic> gene mutations, which have never been reported before as a relevant gene mutation for congenital hearing loss related with stapedial fixation. Under the possible diagnosis of congenital stapedial fixation with mixed hearing loss caused by <italic>NOG</italic> gene mutations, the patient has started to wear bilateral hearing aids and is being followed up for possible surgical therapy. Here, we report this case of congenital mixed hearing loss caused by novel <italic>NOG</italic> gene mutations with a review of the literatures.</p></trans-abstract>

<title>서 론</title>
<p>소아 난청 유병률은 1000명당 1&#x0002d;6명으로 대부분 신생아 시기에 발생한다&#x0005b;<xref ref-type="bibr" rid="b1-kjorl-hns-2022-00458">1</xref>&#x0005d;. 소아 난청은 다양한 원인에 의해 발생하지만 50&#x00025;는 유전성 소인을 고려해야 하며, 그 외 20&#x00025;&#x0002d;25&#x00025;는 임신 중, 출생 또는 출생 후 환경적 요인이 영향을 미친다&#x0005b;<xref ref-type="bibr" rid="b2-kjorl-hns-2022-00458">2</xref>&#x0005d;. 유전성 난청의 경우 80&#x00025;는 상염색체 열성의 유전 형태, 20&#x00025;는 상염색체 우성의 유전 형태, 1&#x00025;는 미토콘드리아 유전 형태를 가진다. 또한 동반 증상에 따라 크게 증후군형과 비증후 군형으로 구분 가능하며, 증후군형의 경우 다른 장기의 기형 및 장애가 동반되며, 비증후군형의 경우 청력 감소의 증상을 나타낸다. 유전성 난청의 70&#x00025;에서 비증후군형을 보인다&#x0005b;<xref ref-type="bibr" rid="b3-kjorl-hns-2022-00458">3</xref>,<xref ref-type="bibr" rid="b4-kjorl-hns-2022-00458">4</xref>&#x0005d;.</p>
<p>등골 강직(stapes ankylosis)은 <italic>NOG</italic> 유전자의 변이에 의해 발생하는 우성 유전병으로, 다양한 형태의 표현형을 가지는 증후군성 유전성 난청이다. <italic>NOG</italic> 유전자는 noggin이라는 단백질을 암호화하며, 이 단백질은 뼈와 관절의 발달에 중요한 역할을 한다. <italic>NOG</italic> 유전자의 변이는 청력 상실과, 원시안, 무딘 엄지손가락과 큰 발가락 및 그 외 골격계 이상 등의 다양한 신체적 특징을 보인다&#x0005b;<xref ref-type="bibr" rid="b5-kjorl-hns-2022-00458">5</xref>&#x0005d;. 상염색체 우성의 선천성 등골 강직 환자들은 생후 청각장애 증상이 나타나며, 성장하면서 더 악화되지는 않는 특징을 보인다.</p>
<p>저자들은 최근, 기존에 보고된 바 없는 NOG 유전자 돌연변이가 확인된 양측성 혼합성 난청 소견을 보인 환아 1예를 경험하였기에 문헌 고찰과 함께 보고하고자 한다.</p>
</sec>
<sec sec-type="cases">
<title>증 례</title>
<p>특이 기저질환이 없는 6세 남아가 내원 2년 전 발견된 양측 청각장애를 주소로 본원 이비인후과에 내원하였다. 환아는 내원 2년 전 타병원에서 시행한 청력검사상 청력감소 소견을 진단 받았으나 특별한 치료없이 경과 관찰 중이었다. 환자는 문진에서 이충만감, 이명, 이통, 이루 등의 증상은 호소하지 않았다. 가족력상 환자의 친할머니, 어머니, 남동생 및 여동생 역시 청력저하 증상이 있었다(<xref rid="f1-kjorl-hns-2022-00458" ref-type="fig">Fig. 1</xref>). 신체 검진에서 환아는 원시안으로 안경 착용 중에 있었으나, 무딘 엄지손가락과 큰 발가락, 그 외 골격계 이상 등 기존에 보고된 바 있는 <italic>NOG</italic> 유전자 돌연변이와 관계된 신체적 특징은 관찰되지 않았다. 양측 고막은 정상소견을 보였다.</p>
<p>순음청력검사(pure tone audiometry)에서 우측 기도, 골도 각각 60 dB HL, 35 dB HL, 좌측 기도, 골도 각각 55 dB HL, 35 dB HL로 공기&#x0002d;뼈전도 차이(air&#x0002d;bone gap)가 양측 모두 20 dB HL 이상 확인되는 혼합성 난청이 관찰되었다. 어음청취역치 검사(speech reception threshold test)에서 청력역치는 우측 60 dB HL, 좌측 50 dB HL에서 관찰되었으며, 어음 명료도(speech discrimination score)는 85 dB에서 우측 48&#x00025;, 좌측 52&#x00025;로 확인되어 혼합성 난청으로 확인되었다(<xref rid="f2-kjorl-hns-2022-00458" ref-type="fig">Fig. 2A and B</xref>). 등골근반사검사에서 등골 반응이 양측 모두에서 관찰되지 않았고, 변조 이음향방사검사에서도 양측 모두에서 반응을 보이지 않았다(<xref rid="f2-kjorl-hns-2022-00458" ref-type="fig">Fig. 2C and D</xref>). 비조영 측두골 컴퓨터단층촬영(non&#x0002d;enhanced temporal bone CT)에서 등골 주변이나 fissula ante fenestram에 이경화증을 의심할만한 음영 등의 특이소견은 관찰되지 않았다(<xref rid="f3-kjorl-hns-2022-00458" ref-type="fig">Fig. 3</xref>). 본 환아의 난청 원인을 찾고자 시행한 차세대염기서열기법의 난청 유전자 패널 검사에서 <italic>NOG</italic> 유전자의 exon 1에서 기존에 보고된 적이 없는 돌연변이 c.509C&#x0003e;T가 이형접합의 형태로 관찰되었다. 본 유전자 돌연변이는 population database에서 보고된 적이 없는 변이이며, 가족 유전자 검사를 시행한 결과 청력저하의 소견을 보이는 가족구성원들(남동생, 여동생)에서도 동일한 유전자 변이를 관찰할 수 있었다. 관찰되는 혼합성 난청에 대해 환아는 귀걸이형 보청기를 양측 귀에 처방받고 청각재활 및 정기적인 추적 관찰을 시작하였으며, 청각 재활이 미비하거나 난청이 진행할 경우 조기 시험적 고실개방술을 통한 등골절개술을 고려하면서 외래 경과 관찰 중이다.</p>

<sec sec-type="discussion">
<title>고 찰</title>
<p>전음성 난청을 가진 소아 환자 중 선천성 등골 고정의 빈도는 국외에서 0.5&#x00025;&#x0002d;1.2&#x00025;로 알려져 있으나&#x0005b;<xref ref-type="bibr" rid="b6-kjorl-hns-2022-00458">6</xref>&#x0005d;, 국내에서는 그 빈도가 아직까지 잘 알려지지 않았다. 소아 난청의 원인이 될 수 있는 선천성 등골 고정이 의심되는 환아의 정확한 평가를 위해서는 철저한 병력과 가족력의 파악이 중요하다. 또한, 청력 저하의 발현 시기, 양측성 유무, 진행 형태, 돌발성 및 변동성 유무 등을 파악해야 하며, 등골 강직과 연관된 증후군성 증상의 유무를 확인하기 위해 두개 안면부 기형, 안과적 질환 등을 포함한 전신 진찰도 시행해야 한다. 외이도 폐쇄(canal atresia), 심실중격결손(ventricular septal defect), 이전부 누공(preauricular fistula), 제2새열누공(second brachial cleft)도 동반될 수 있다고 알려져 있으므로 검사를 통해 확인할 필요가 있다&#x0005b;<xref ref-type="bibr" rid="b7-kjorl-hns-2022-00458">7</xref>&#x0005d;. 가족력 여부가 선천성 등골 고정의 진단에 도움이 되지만, 뚜렷한 가족력이 없다고 해서 선천성 등골 고정의 진단을 배제할 수는 없다. 비진행성 난청을 호소하는 소아 중 전음성 난청을 보이며 신체 검진상 고막에서 정상 소견을 보일 시 선천성 등골 고정을 우선적으로 고려해 볼 수 있겠다. 진행성 전음성 난청일 경우 가장 흔한 이경화증(otosclerosis)을 우선 고려하지만, 이 경우 발생 연령과 진행성 난청 등의 임상 양상이 감별에 도움을 준다. 15세 이하의 소아에서 기도-골도 청력 차이가 50 dB HL 정도이며, 골전도 감소 소견이 없는 전음성 난청의 경우 우선적으로 선천성 등골 고정을 의심해 보아야 한다.</p>
<p>선천성 등골 고정이 상염색체 우성 유전일 경우 대립 유전자가 하나만 있어도 증상이 발현된다. 성별의 차이는 없으며, 자손의 반수에서 나타나는 수직형 가계도(vertical pedigree)를 보이게 된다. <italic>NOG</italic> 유전자는 noggin이라 불리는 단백질을 발현하는데, 이 단백질은 bone morphogenetic proteins (BMPs)에 부착하여 불활성화 시키는 역할을 가진다. BMPs는 transforming growth factor&#x0002d;&#x003b1;에 속하는 신호 단백질(signaling protein)이다. 미로골낭(otic capsule)과 등골의 골화부분이 발생과정에서 가늘어지며, 등골 주위의 윤상 인대의 탈골화가 진행됨에 따라 움직임을 가지게 되는데, <italic>NOG</italic> 변이가 발생할 경우 이 과정에 문제가 발생하여 등골 고정이 발현되는 것으로 보고되고 있다&#x0005b;<xref ref-type="bibr" rid="b8-kjorl-hns-2022-00458">8</xref>&#x0005d;.</p>
<p>선천성 등골 고정의 수술적 치료로서 등골개창술, 등골절제술을 시행할 수 있으나 외림프액의 유출, 이명, 현훈, 골도 청력역치의 악화 등의 합병증이 발생할 수 있어 적절한 수술 시기를 선택할 필요가 있다. 수술 시 등골 주위의 혈관 생성 저하, 등골판의 비후, 등골판이 주변의 골미로낭과 연결되어 윤상 인대가 보이지 않는 상태 등이 관찰된다&#x0005b;<xref ref-type="bibr" rid="b8-kjorl-hns-2022-00458">8</xref>,<xref ref-type="bibr" rid="b9-kjorl-hns-2022-00458">9</xref>&#x0005d;.</p>
<p>본 증례의 환아는 비교적 이른 시기인 6세에 발견된 선천성 혼합성 난청으로, <italic>NOG</italic> 유전자 변이와 관련된 선천성 등골 고정이 의심되고 일반적인 전음성 난청에 비해 심각한 혼합성 난청을 보이고 있었다. <italic>NOG</italic> 유전자 중 기존에 보고된 바 없는 exon 1의 c.509C&#x0003e;T가 이형접합 변이를 관찰할 수 있었던 본 환아에게 청각 재활을 위해 우선 보청기 착용을 시행하였으며, 추적 관찰 중이고, 난청의 진행 및 청각 재활 경과에 따라 필요 시 조기 수술적 치료도 고려하고 있다.</p>
<p>저자들은 양측 귀의 선천성 혼합성 난청이 관찰된 환아에서 시행한 유전자 검사 결과 기존에 보고된 바 없는 혼합성 난청을 동반한 <italic>NOG</italic> 유전자 돌연변이를 확인하였기에 유전성 난청의 가계도를 지닌 선천성 등골 고정에 의한 혼합성 난청으로 추정되는 본 환아의 증례를 문헌 고찰과 함께 보고하는 바이다.</p>
