Signs and symptoms are caused by local expansion of the mucocele and include headache(the most common symptom), ophthalmoplegia, visual disturbance and proptosis. Ophthalmoplegia and visual disturbance that are usually gradual in onset, but may be sudden, may complicate the illness. Moreover, the sphenoid mucocele causing sudden onset of blindness and hyperprolactinemia is an extremely unusual condition. We experienced a huge mucocele involving the sphenoid sinus manifested a sudden onset of blindness and hyperprolactinemia. The case was treated with sublabial transseptal approach and sphenoid sinusotomy successfully. So, we present this case with a brief review of the literature. </p></span></font>
