The authors observed on clinical status of 127 unilateral vocal cord paralysis from April, 1983 to April, 1992 at Department of Otorhinolaryngology, Yongdong Severance hospital, Yonsei University College of Medicine. The results were as follows ; 1) In sex distribution, male were 73 cases and female 54 cases, so the male to female ratio was 1.4 : 1. By age distribution, most of the cases were from 3rd to 7th decade. 2) In chief complaints, most of the cases(101 cases ; 79.5%) had hoarseness only. Aspiration, stridor, cough, dysphagia were in some cases. 3) In laterality of the paralysed vocal cord, 94 cases were in the left cord, 33 cases in the right cord, so the left to right ratio was 2.7 : 1. 4) In etiology, 59 cases(46.5%) were idiopathic, 18 cases by surgical trauma, 19 cases by neoplasm, 10 cases by non-surgical trauma, 7 cases by the tuberculosis, and 7 cases by the endotracheal intubation. 5) In the position of paralysed vocal cord, most of the cases(104 cases ; 81.9%) were in the paramedian position. 6) Cord atrophy, arytenoid rotation and contralateral cord compensation were correlated with the duration of vocal cord paralysis.
