A Clinical Study of Vestibular Neuritis

ABSTRACT

Eighteen patients with acute vertigo subsequently diagnosed as vestibular neuritis from February 1987 to August 1988 were analysed on detailed clinical and electronystagmographic findings in Kyung Hee medical center. The following results were obtained ; 1) Men were afflicted somewhat less frequently than women in the ratio of 4 : 5(8 men ; 10 women). By age distribution, most of patients(16 cases, 88.9%) were from 3rd decade to 6th decade and its highest incidence was at 5th decade(8 cases, 44.4%). 2) Spontaneous nystagmus was demonstrated in 12 cases(66.7%) during the period of vertigo. Simultaneous caloric testing showed diminished sensitivity of the labyrinth of right side in 11 cases(61.1%). Cases of bilateral involvement were not ecountered. 3) In 15 cases(83.3%) the onset of the vertigo was sudden, and in 13 cases(72.2%) sensations of rotation were experienced. In 13 cases(72.2%) the vertigo was limited to a single attack while in 5 cases(27.8%) the attacks had been repeated. Unconsciousness during the vertigo was encounted in only 1 case(5.6%). 4) There were nausea in 12 cases(66.7%), vomiting in 11 cases(61.1%), tinnitus in 5 cases(27.8%), headache in 5 cases(27.8%) during the vertigo. 5) The onset of vestibular neuritis were found to be clustered in the early spring and winter(10 cases, 55.6%). 6) A total of 4 cases(22.2%) reported a preceding or simultaneous infection as follows; upper, probably viral respiratory infection(2 cases, 11.1%), pulmonary tuberculosis(1 case, 5.6%) and gastroenteritis(1 case, 5.6%) 7) In all cases the tympanic membranes were normal. Audiograms recorded during the attack of vertigo were normal, or showed unilateral or bilateral changes of different origin : NIHL, presbycusis. The findings on ABR were normal in all cases. 8) After varying intervals 16 cases(88.9%) became symptom-free, and no complications were observed(mean duration of illness ; 7 weeks). 9) The mean time between onset of vertigo and hospitalization was 3.7 days.
