Although the Caldwell-Luc procedure has been fundamental in the treatment of chronic paranasal sinusitis, high complication rate has been a main disadvantage of the procedure. There complications include facial swelling, hemorrhage, cheek discomfort, and facial and tooth paresthesia. In order to avoid these complications, modifications of this standard procedure were designed according to exact anatomical knowledge. Modifications include 1) modified incision of the gingivobuccal area, 2) a small opening in the lateral margin of the canine fossa, 3) use of electric drill, 4) partial resection of antral mucosa, 5) middle meatal antrostomy, 6) use of pneumatic balloon, and 7) covering of the facial opening with a homologous dura.
