We report a case of traumatic CSF otorrhea associated with transverse temporal bone fracture. This case has Mondini dysplasia, also. Iopamidol CT scan was used to localize the site of the leakage, and showed the dye in the vestibule, mesotympanum and eustachian tube. Using the transmastoid extracranial approach, the site of the leakage was sucessfully repaired with temporalis muscle, fascia and histoacrylic glue. We discussed the diagnostic approach of the CSF otorhinorrhea and it's management.
