Perichondritis and It's Treatment |

ABSTRACT |
Perichondritis is a rare condition that otologist encounters occasionally, but it is troublesome in our work. Auricular cartilage is a primitive tissue and infection of perichondrium causes interference with the blood supply and subsequent loss of nourishment to the cartilage, so that this can eventually result in loss of tissue and deformity. The authors insisted that the principle of the treatment are as follows ; 1) pain control, 2) shortening the period of disease's course and 3) prevention of residual deformity. The authors treated the perichondritis by the following methods ; 1) irrigation with antibiotics, 2) irrigation with Nelaton catheter and antibiotics, 3) necrotomy(resection of the involved cartilage) and 4) ultraviolet radiation. The authors proposed that the choice of method on the treatment of perichondritis is according to the involved site of auricle. When it was involved the cartilage with open wounded case, it could be cured with simple irrigation as like the No. 1 case (Fig.1). When it was involved the cartilage of helix and it's necrosis, the necrotomy should be done as like the No. 4 & 5 cases (Fig. 7). When the particular site was involved on the cavum concha without involving helix, partial necrotomy with catheter irrigation through the inserting catheter should be done as like the No. 2 cases ((Fig.2), 3, 4 & 5). When it was only involved to the cartilage of crus helicis, it could be cured with necrotomy at the crus helicis of involved cartilage as like the No. 3 cases (Fig.4) in a early attempts.
