Clinical Analysis on Transseptal Transsphenoidal Approach to Pituitary Gland |

ABSTRACT |
42 patients were analyzed who had received transseptal transsphenoidal surgery on pituitary lesions from June 1977 to April 1984 at Seoul national university hospital. Of 42 cases, 35(83%) were functioning pituitary adenomas, 6 were nonfunctioning and 1 was CSF rhinrrhea. The predominent symptoms were amenorrhea and galactorrhea(26%), headache(24%) acral enlargement(21%) and disturbances of visual field(12%). Of 14 cases with preoperative visual field disturbance, 6 cases(43%) were improved with operation, and 32 cases(91%) of 35 functioning pituitary adenomas were improved endocrinologically. Surgical mortality was zero. Complications associated with operation were CSF rhinorrhea(4 cases ; 9.5%), transient diabetes insipidus(3 cases ; 7%) and each 1 case of meningitis, septal perforation, rhinitis sicca, total unilateral blindness and transient respiratory distress.
