ABSTRACT

Ludwig's angina is a potentially lethal, rapidly spreading cellulitis involving the sublingual and submaxillary spaces. In 1836, Wilhelm von Ludwig described clinical and necropsy observations, but the danger of insidious airway obstruction has been appreciated since antiquity with specific references made by Hippocrates, Galen, etc. In preantibiotic period, mortality rate exceeded 50%, but there have been much reduced mortality rate and incidence because of wide-spread use of antibiotics in orodental infection, improved dental care, as well as adherence to strict diagnostic criteria. Early and aggressive use of appropriate antibiotics and protection of the airway are the mainstays of a successful treatment regimen. Judicious surgical intervention is indicated in those patients who develop localized abscesses while on antibiotics are unresponsive to medical treatment. The authors experienced 7 cases of Ludwig's angina and presented the cases with review of literatures.
