ABSTRACT |
One hundred and twenty five cases of transseptal transsphenoidal hypophysectomies during a period of ten years from June, 1977 to March, 1988 were analysed. The results were as follows : 1) Male to female ratio was 1 to 1.71 2) One hundred and eighteen cases(94.4%) were functioning adenomas, 33 cases(26.4%) nonfunctioning adenomas and the remaining others, Prolactin secreting adenoma was the most common of the functioning adenomas. 3) The chief complaints were headache(28.0%), visual disturbance(23.2%), amenorrhea(22.4%), acromegaly(12.8%) and galactorrhea(7.2%) in order of frequencies. 4) Pneumatization of the sphenoid sinus was sellar(68%), presellar(20.8%) and unclassifiable(11.2%). 5) Destruction of the sellar floor was observed radiologically in 55 cases(44.0%) and thickening(>2mm) in 16 cases(12.8%). 6) Of 68 cases with preoperative visual disturban, 35 cases(51.5%) were improved and 8 cases(11.7%) were aggravated after operation. 7) Postoperative complications were transient diabetes insipidus(18.4%), panhypopituitarism(5.6%), CSF rhinorrhea(4.8%), meningitis(4.0%) and septal perforation(2.4%).
