ABSTRACT |
The flap is commonly used for reconstruction of the surgical defect in the head and neck area. The defect of head and neck area has been managed by the use of skin graft and local, regional or distant flaps. Various myocutaneous flaps have been developed recently and pectoralis major, trapezius. Sternocleidomastoid or latissimus dorsi flaps have been frequently used for the surgical defects in functional and cosmetic aspects. Fifty major and minor flaps used for reconstruction of the head and neck defects during a period of 5 years from July, 1983 to August, 1988 were analyzed. The results were as follows : 1) Twenty cases were reconstructed with skin flaps and 30 cases were myocutaneous flaps, of which 26 cases were pectoralis major myocutaneous flaps. Twenty skin flaps were 12 deltopectoral flaps, 1 forehead flap, 6 advancement flaps and 1 rotation flap. 2) Thirty eight cases of flaps were completely viable without any necrosis., There was total necrosis of the flap in 4 cases and partial necrosis in 8 cases. 3) Complications such as wound infection and disruption or fistula formation were more common in the patients who received radiation therapy preoperatively. 4) Flap viability more than 75% was 92%.
