Clinical Evaluation of Extranodal Malignant Lymphoma in ENT Field

ABSTRACT

Malignant lymphoma is considered tumor of the immune system, including tumor of lymphocyte and Hodgkin's disease. They rarely include tumor derived from the histiocyte, which may arise in any organs and glands in the body. It is mainly found in the cervical, axillary and inguinal lymph node, but gastrointestinal tract, palatine tonsil, nasopharynx, oropharynx, nasal cavity and paranasal sinus, and salivary glands may be involved. The authors reviewed the chart and tissue biopsy slides of 24 cases of extranodal malignant lymphoma in ENT field, which were confirmed with tissue biopsy or needle aspiration biopsy, among the patients visiting at ENT department of Hang Yang University Hospital from March, 1981 to June, 1985. The results were following ; 1) The sex ratio of male and female was 1 : 1.1. 2) The commonest age groups were 3rd decades and 4th decades in 5 cases(20.8%) and mean average age was 40.4 years old. 3) Predominant symptoms were swallowing difficulty in 9 cases(37.5%), nasal obstruction in 5 cases(20.8%), nasal bleeding in 2 cases(8.3%), swelling of submandibular gland in 2 cases(8.3%), orbital swelling in 1 cases(4.2%). 4) Predominant sites were palatine tonsil in 14 cases(58.3%), nasal cavity and paranasal sinus in 6 cases(25.0%), nasopharynx in 2 cases(8.3%), submandibular gland in 2 cases(8.3%). 5) According to Rapparport classification, the commonest histopathologic type was histiocytic type in 14 cases(58.3%), and the others were mixed type in 6 cases(25.0%), lymphoblastic type in 1 case(4.2%) and 3 cases were not histopathologically confirmed. 6) The commonest therapeutic trial was combined chemotherapy and radiotherapy in 6 cases(25.0%), chemotherapy alone in 5 cases(20.8%), radiotherapy alone in 5 cases(20.8%), radiotherapy after surgery in 3 cases(12.5%), surgery alone in 1 case(4.2%) and refusal to treatment in 4 cases. 7) Treatment results after 3 years follow up were alive with no evidence of disease in 10 cases(55.6%), alive with local recurrence or lymph node involvement in 5 cases(27.8%), expired due to sepsis or hepatorenal failure in 3 cases(16.7%), and unable to follow up in 6 cases.
