The Significance of Ultrasound Examination in the Diagnosis of Nasal Bone Fracture

ABSTRACT

Nasal bone fracture is commonly misdiagnosed or inadequately treated, because false negative rate of the radiographic diagnosis reaches about 50%. This is not only because of difficulty in differentiation between suture lines and vascular groove but also bacause of overlapping appearance of both nasal bones on radiographic findings. For this reason, physical examination was regarded as the most important method in diagnosis of nasal bone fracture. Recently, ultrasound examination has been performed in several ENT field due to its reliability and safety. The authors investigated the clinical significance of ultrasound examination in the diagnosis of nasal bone fracture. In 24 cases of nasal bone fracture patients, the results are as follows : 1) The main symptoms and signs were tenderness in nasal dorsum(88.9%), and nasal bleeding(75%). 2) The most common cause of trauma was accident(12 cases, 50%). 3) The most common type of nasal bone fracture was depressed fracture(14 cases, 58.3%). 4) False negative rate of plain x-ray and ultrasound examination was 21.9%(7 cases) and 20.8%(5 cases), respectively. 5) Deteciton rate of fracture side of plain x-ray and ultrasound examination were 23.5% and 100%, respectively. 6) Overall diagnostic accuracy of ultrasound examination was 70.8%. In conclusion, ultrasound examination was a helpful adjunct in diagnosis of nasal bone fracture.
