Postoperative Delayed Facial Palsy in Chronic Otitis Media

ABSTRACT

From September 1993 to June 1995, 332 cases of chronic otitis media with or without cholesteatoma were operated by one operator and seven cases(2.1%) of delayed facial palsy were developed postoperatively. Occurrence of the delayed facial palsy were more common in the female patients(6/7) and predominant in the sixth decade female ; 17%(4/23) of the operated sixth decade female patients were developed delayed facial palsy. Onset of delayed palsy was varied form second to twelfth postoperative day and the degree of the palsy were varied from House-Brackmann grade II-IV. Among the seven delayed facial palsy, mastoidectomy were performed in five, and atticotomy in two, respectively. Among the 332 operated patients, 60(18.1%) patients were exposed the facial nerve and three oh them developed delayed facial palsy. The delayed facial palsy was significantly more common in the facial nerve exposed patients to compare with the unexposed patients, although there were no direct damage to the facial nerve in those who developed delayed facial palsy. All the seven delayed palsy patients were treated conservatively as like a Bell's palsy and all patients were recovered completely within about 9 weeks postoperatively.
