Clinical Observation of the Laryngomicrosurgery

ABSTRACT

The combined use of the operating microscope and suspension laryngoscope permitted laryngomicrosurgery. The introduction of endolaryngeal microsurgery by Kleinsasser in 1965 made more useful approach for laryngeal surgery and diagnosis. The technique has advanced the examination of the static larynx and the performance of endolaryngeal operations and has gained widespread acceptance. Laryngomicrosurgery was performed on 24 cases with various laryngeal lesions at the dept. of Otolaryngology, Chonnam University Medical School during the period of 20 months from January 1977 to August 1978. And the following results were obtained. 1) Distribution of disease in order of frequency were vocal polyp and nodule(12 cases, 50.0%), papilloma(6 cases, 25.0%), cancer(3 cases, 12.5%), trauma(2 cases, 8.3%) and web formation(1 case, 4.2%). 2) Sex ratio was 1.7 : 1, being 15 cases of male and 9 cases of female. 3) The age distribution showed a predilection on third decade(8 cases, 33.3%). The eldest age 68 and youngest age was 4 respectively. 4) The laryngomicrosurgery were performed on 20 cases(83.3%) for surgical treatment and 4 cases(16.7%) for examination. 5) Laryngomicrosurgery were performed under the endotracheal general anesthesia on 21 cases(87.5%) and local anesthesia on 3 cases(12.5%). 6) Maximum phonation time(MPT) was measured before and after operation in 6 cases(25.0%). As a rule, the larger pathologic lesion showed the better MPT score, the smaller pathologic lesion showed the worse MPT score.
