Conservation Surgery for Laryngeal Tumor |

ABSTRACT |
Conservation surgery for laryngeal tumor has been developed and performed frequently. Conservation surgery of the larynx deals specifically with the goal for complete tumor excision while maintaining respiration, deglutition, and voice production. 25 cases of laryngeal tumor which were treated with conservation surgery at Department of otolaryngology of Kyungpook National University Hospital from June 1984 to July 1988. Analysis is presented of the histopathologic and clinical features, type of surgery, postoperative complication, recurrence, and etc. In histopathologic study, 22 cases(88%) are squamous cell carcinoma, malignant fibrous histiocytoma 1 case(4%), neurilemmoma 1 case(4%), pleomorphic adenoma 1 case(4%). Horizontal partial laryngectomy was performed in 17 cases(68%), and vertical partial laryngectomy 8 cases(32%). In 22 cases of squamous cell carcinoma, 7 cases have metastasis microscopically but clinically 5 cases have cervical metastasis. Among 3 cases which don't have cervical metastasis clinically, treated with elective neck dissection, 2 cases metastasize neck microscopically. During follow up postoperatively, there are 5 cases(35.7%) of recurrence in supraglottic cancer, of which 4 cases have neck recurrence and 1 case has neck and local recurrence. Postoperative death is 2 cases. Elective neck dissection is recommended in conservation surgery of the larynx for
T2 & T3 supraglottic cancer.
