Management of Irradiated Fixed Neck Nodes |

ABSTRACT |
Fixed neck nodes have unfavorable prognosis due to their extracapsular extension and local spread. Therefore the management of these neck nodes are an integral of the overall treatment of patients with head and neck cancer. Fixed neck nodes can be treated by various types of neck dissections, irradiation or a combination of both and still pose a therapeutic dilemma. We anlaysed the outcome of neck dissections performed on 26 patients who had irradiated fixed neck nodes during the past 10 years. The results are as follows. 1) We obtained 65.4% of free resection margin in the irradiated fixed neck nodes. 2) Postoperative median survival time of patients with irradiated fixed neck nodes was 10.6 months and there was no influence by the type of preoperative irradiation. 3) The limited neck dissection group had more NED and less postoperative complications than the radical neck dissection group. In conclusion, fixed nodes are amenable to resection following preoperative irradiation and the limited neck dissection could be useful to manage the irradiated fixed neck nodes.
