편도와의 악성종양을 절제하기 위해 다음과 같이 원발병소 절제술을 시행하였다. 수술 시행 시 손상에 주의해야 하는 구조물에 해당하지 않은 것은?

① 설하신경(hypoglossal nerve)

② 상후두신경(superior laryngeal nerve)

③ 설동맥(lingual artery)

④ 내경동맥(internal carotid artery)

⑤ 횡경동맥(transverse cervical artery)

해 설 사진과 같이 외측인두절개술(lateral pharyngotomy)을 통해 편도와에 접근할 경우 설하신경, 상후두신경 및 설동맥의 손상을 피하기 위해 주의해야 하며, 편도와의 내하방에 존재하는 내경동맥의 손상에 주의해야 한다. 횡경동맥의 경우, 4경부구역에 존재하여 편도와 절제와는 무관하다.
