Recently, the temporal bone trauma have been increased because of the industrial development and traffic accidents. But, there have been no reports that external, middle, inner ear and facial nerve were injured by penetrating trauma throught the external auditory canal. We experienced a case of otologic trauma which the external, middle and inner ear were destroyed with facial palsy and CSF otorrhea by the penetration of a metallic ball-point pen through the external auditory canal. The metallic ball-point pen was removed successfully through open cavity mastoidectomy approach and CSF otorrhea was repaired with temporalis fascia and bone chip. The facial nerve was grafted with sural nerve after two months from the first operation.
