The Results of Inhalant Allergy Tests and Treatment

ABSTRACT

Recently there has appeared an increased tendency to understand allergic diseases in the ENT field. Attention to allergies of the nose and paranasal sinuses, pharynx, larynx and bronchial tree, external and middle ear, E-tube, cochlear vestibular system, CNS, and oral cavity is important to cure the diseases, the etiology of which is uncertain. Since 1976, at allergy Clinic of Presbyterian Medical Center, Jeonju, Korea, the Provocative Food Allergy Test and the Inhalant Allergy Test by the Serial dilution titration technique(proposed by Herbert Rinkel) have been performed for evaluation and management of allergic disorders. In a previous article in 1978, we reported on the diagnosis of Inhalant Allergy Tests on the basis of the results of allergy tests for one year from September, 1976. Now, we are reporting the results of Inhalant Allergy tests and treatment from October, 1977 to December, 1980 : 1) Among the total of 314 cases, the male patients numbered 176 and the female patients, 138. On age distribution, the majority(240 cases, 76%) were elongated to the age group 11-40. 2) On the test methods, 53 cases received preseasonal tests, 79 cases received coseasonal tests, and 182 cases received perenninal tests. 3) The distribution of the patients' chief complaints was respiratory symptoms(71%), dermatologic symptoms(22%), gastrointestinal symptoms(3%), and others. The seasonal distribution of the symptoms was perennial(57%), winter(22%), summer(9%), autumn(7%) and spring(5%). 4) The order of the symptoms due to allergens which showed 'end point of reaction' was as follows : a) Pollen ; nasal symptoms-dermatologic symptoms-headache-asthmatic cough-gastrointestinal symptoms-others. b) Mold ; dermatologic symptoms-nasal symptoms-asthmatic cough-headache-gastrointestinal symtoms-others. c) House dust ; nasal symptoms-asthmatic cough-dermatologic symptoms-headache-gastrointestinal symptoms-others. d) Mixed allergen ; nasal symptoms-dermatologic symptoms-asthmatic cough gastrointerstinal symptoms-others. e) Total ; nasal symptoms-asthmatic cough-dermatologic symptoms-headache-gastrointestinal symptoms-others. 5) Among the 314 cases, the number of cases which showed 'end point of reaction' on the various allergens were :

a. House dust   178 cases
b. Pollen- Mesquite 170 cases
  Chenopod 100 cases
  Red maple 091 cases
  Ragweed 090 cases
  Grass 086 cases
  Cottonwood 081 cases
  Black walnut 080 cases
  Desert ragweed 079 cases
c. Mold- T.O.E. 148 cases
  Mold mixture A 132 cases
  Mold mixture B 131 cases
  Mold mixture C 124 cases
  Alternaria 079 cases

6) The number of 'end point of reaction' on various allergens and the solution number showed as follows :

a. Pollen- No. 2 solution 975 (51%)
    (1927) No. 3 solution 665 (35%)
  No. 4 solution 202 (10%)
b. Mold- No. 2 solution 371 (55%)
    (667) No. 3 solution 208 (31%)
  No. 4 solution 64 (10%)
c. House dust- No. 2 solution 90 (51%)
    (175) No. 3 solution 68 (39%)
d. Dander- No. 2 solution 14 (70%)
    (20) No. 3 solution 5 (25%)
e. Total No. 2 solution 450 (52%)
    (2791) No. 3 solution 946 (34%)
  No. 4 solution 281 (10%)

7) Ninety-five among the 314 cases received a follow-up check, and of these, eighty-one cases(85%) responded well to immunotherapy. Among these, 31 cases responded within 6 months. 40 cases responded within 6 to 12 months. 7 cases responded within 12 to 18 months. 3 cases responded within 18 to 24 months.
8) The number of the changes of 'end point of reaction' of the allergens on the retitration patients (22 cases) were as follows ;

a. on 7 cases, retitrated within 6 months,
    shift to right 16
    no shift 17
    shift to left 08
b. on 15 cases, retitrated within 6 to 12 months
    shift to right  08
    no shift 37
    shift to left 09
